This is a small sample of some of my work.  I have been struggling to place an example of the work that I can do, and it occurred to a friend of mine that I had this Diabetes Tracker database that I envisioned, designed, programmed, and researched. 

I am NOT a medical professional, just a person with diabetes.  If you have suggestions that might add to this database and it's functionality, your input would be greatly appreciated.

The need to track important data easily when you have a disease such as Diabetes is very important.

The database allows you to track:

  • Blood Sugar Levels
  • Medications Prescribed
  • Medications Taken (over the counter)
  • Insulin Injected in units
  • Daily Diary - Where you can note whatever you wish.
  • Meals - there are 7600+ different foods in the database from the FDA, the nutritional data.  That data uses the Carbohydrates to help you determine the amount of insulin to take. 
  • Foods - Adding new foods is easy, the form looks almost exactly like the nutritional panel found on foods in the US. 
  • Blood Pressure/Pulse - This is something that some of us check regularly.
  • Exercise - There is a fairly useful list of exercises, including those like housework, knitting, sewing, etc.

The database includes some reports also that I have designed to make it easier to show the doctor what I have been doing.  Normally, the doctor is happy just seeing the Insulin Intake and Blood Sugars in a graph format.  There could be other reports added to this. 

I am not selling this database, it is not complete enough, and does not have a Doctor's direct input on the handling of the data.  If you download this database, it's still my own work, and would like at least to be told that is being used somewhere.  If you change it and break it, I don't support the database.  The database design is my own, and I must be notified if it is going to be sold, included in any type of treatment package.

You MUST be running Access 2000, Access 2000 or Access XP (Access 2002) to use this database.  There are no current versions in previous versions of Access at this time. 

A reminder, this is a personal database that I have designed to meet the needs of myself and my own health care provider.  It is presented as is with NO warranties for it's use.  There are no security measures in the database that would stop you from viewing or changing the design of the database.

This database should at the very least show you the style in which I design forms, some of the calculations being processed to calculate Insulin according to settings in the Database Ownership.  I recommend highly checking with you doctor before putting this database to work for you.  I want this to be a help, not a hinderence to people.  And it gives me a sample that I can show someone of the kind of work that I do.

NOTICE:  This database is NOT finished,  The functions in the database have been designed as the need arises and has not been designed as an all inclusive tool for the general public.
